Photo & Video Release

I hereby irrevocably grant the Suzuki Music Association of California (SMAC) permission to distribute, transmit, publish, copy, or otherwise exploit, either in whole or in part, either digitally or any other medium now known or later discovered, photographs, videos or audio recordings of my minor child or wherein he/she may appear, including composite or artistic forms and media, and those submitted by me to SMAC for any purpose. I understand and agree that the medium in any form may be used and exploited with or without identifying my child. I understand that the images may be altered or modified in any manner. I waive any rights to intellectural property, royalties or other compensation arising from or related to the use of the photos or videos. I waive any right to inspect or approve the finished photos/videos or printed or electronic matter that may be used in conjunction with them now or in the future, whether that use is known or unkown to me.

I hereby hold harmless and release and forever discharge SMAC from all claims, demands and causes of action which I, my heirs, representatives, executors, administrators or any other person acting on my behalf or on behalf of my estate have or may have by reason of this authorization. I have read this document and fully understand and agree to its contents.

By submitting this form electronically, I hereby certify that I am the parent or guardian of the minor identified in the registration form in which this agreement exists and do hereby give my consent without reservations to the foregoing on behalf of this person.
