During practice sessions, and events, I understand that I may be included in candid photographs or videos. Pictures or videos may be used in informational or promotional materials, including our website.

I hereby grant my consent to SMAC to photograph and/or electronically record all appearances in or with Los Angeles Suzuki Institute (LASI) 2026 - rehearsals, classes and concerts (collectively SMAC events). I grant SMAC the right to use my image and likeness for distribution at any time in the following SMAC media: fliers, pamphlets, website, and social media.
